Questionnaire for Parents: Individual Education Application
Dear Parents,
Please complete the questionnaire, which is part of the application for your child’s individual education under § 41 of Act No. 561/2004 Coll. (the Education Act). The data provided will be used to assess the application and to ensure the proper implementation of individual education.
What are considered serious reasons under the Ministry of Education regulation?
According to Section 41 of the Education Act and Decree No. 48/2005 Coll., individual education can be permitted only based on serious reasons. The most common serious reasons include:
- Health reasons: The child has long-term health issues, chronic illness, health impairments, or specific educational needs that prevent regular school attendance.
- Family reasons: For example, frequent relocations, long-term stays abroad, or caregiving for a family member.
- Individual educational needs: The child has exceptional talent or, conversely, requires an individual approach due to learning disorders, behavioral issues, or other specific needs.
- Other serious reasons: For instance, specific family circumstances or other conditions that make education in a school impossible.

Individual education is possible only with the school’s consent and all legal conditions must be met. We recommend consulting with the school management before submitting the application.
